STUDENT-ATHLETE BIO FORMS:
1) Click on your sport
2) Download the file (Save the file with your name and sport)
3) Fill out the form
4) Save the file with your name and sport (double check information has been saved)
5) Email file to Sports Information Director John Van Gaston (jvangaston@cerritos.edu)
